Single mother at risk of homelessness experiencing mental and physical disabilities

A single mother who is at risk of homelessness has moved into a new private rental which is costing her more than 50% of her wage. She experiences some mental health distress and also experiences additional physical health issues with a number of diagnosed conditions.
As her finances all go towards rent and utilities she is often unable to afford the basics such as food, toiletries and other large expenses.
Due to her physical health conditions she is unable to drive, her primary mode of transport is walking or public transport which adds additional stress to her other physical health conditions.
Any support that she receives would make a major difference and impact to both her and her sons lives.
