We require a Care Team Co-Ordinator to assist in our Buddy program. The Care Team Coordinator is responsible for providing a seamless care and support service for our clients. This role will match potential clients with buddies and family support buddies to ensure clients are paired up with appropriate support person(s) to provide them with effective advocacy, support, information and representation in their quest for recovery. Through effective operational management and engagement with the team of buddies, this role will ensure compliance with service provision standards and policies and procedures when it comes to client support and services. This role reports to the Office Manager operationally but is also required to work closely with the NDIS.
