Client is displaying motivation and demonstrating proactive behaviours regarding taking steps towards gaining traction and stability post-release from incarceration. Client has several health conditions, including a number of suspected conditions that require diagnosis and treatment to ensure his stability. Client does not currently have access to a mobile phone. This is requested, to ensure his reentry process is smooth and that he is contactable by his support network including case workers and potential employers etc.
