Client and the children separated when client was sentenced to 3 years in prison Since her release client has remained sobber, rebuilt her relationship with her children and has made significant life changes to support her and the children moving forward. Client been staying in a Homelessness shelter and are now in transitional housing with her children Client has shown capacity to engage in case management successfully and work on issues related to homelessness.
Client has depression and anxiety. Having stable transitional family accommodation will assist with this and allow her time to healing and work on her mental health. \Daughter also has lifelong disability with formal diagnosis of autism spectrum disorder (ASD), obsessive compulsive disorder (OCD) and Complex Post- Traumatic Stress Disorder (CPTSD). Family requires some extra items now they are in transitional housing.
