29-year-old has experienced chronic homelessness including rough sleeping, living in crisis accommodation, motels and short hospital stays since 2022. Prior to this, was living with an ex-partner and in the family home. Client engaged in AOD services, including detox, rehab, counselling and pharmacotherapy and has now ceased all support having achieved his goal to cease ALL substance use and pharmacotherapy. Client has now gained full time employment and is needing support to reestablish himself into long term housing.
