Older gentleman living alone without family contact experiencing severe physical and mental health issues. Mainstream health services are unable to maintain care of health issues which cause him significant daily pain. This limits his social interaction with others and within his local community. Applicant is wanting to reengage with supportive community groups and getting out and about in his local community which he misses.
