Since relocating, she has experienced multiple compounding challenges, including the loss of her companion dog shortly after arrival and financial strain related to housing and utilities, limiting her capacity to meet basic living needs.
The client has a history of complex mental health conditions and is currently experiencing significant distress associated with her declining vision and social isolation, with no active family or informal supports.
These combined factors have impacted her ability to maintain adequate personal and household essentials, and support will assist her to remain safe, well and stable in the community.
