22 year old male living in Campbelltown area, is a recipient on the NDIS and has a diagnosis of Mild Intellectual Disability and complex Mental Health. Has a history of Out Of Home Care and limited family supports. His main supports are his NDIS services, support workers and counsellor. Client has a history of drug and alcohol use and recent interaction with police. Due to recent transiency, the young person has lost his phone and has no access to a mobile phone or sim card to contact his supports around his legal proceedings and appointments.
