Many clients who are admitted to our centre present with significant financial hardship and without access to resources. Patients are admitted to our inpatient unit to receive care for their acute mental health presentation and once they are ready for discharge, there is often no way for the community to contact them to provide follow up. This places these individuals at a significant disadvantage and often results in their re-presentation to acute mental health services as they are lost to follow-up and therefore are unable to access community services to aid in their recovery.
