Client has experienced long-term homeless, substance dependence and poor mental health. After finding stability for 6 years including full-time employment and private renting, his mother suddenly passed away, he then lost his job and eventually housing forcing him back into rough sleeping. He is now engaged in an intensive long-term program that will support him into public housing, provide wrap around holistic support as well as an emphasis on social inclusion,
