My client is part of a Program which supports Young People (YP) who are at a young age to make positive changes and divert them away from reoffending. My client has a heart condition - Acute Rheumatic Heart Disease which needs constant monitoring and medication. This young lady has had a tough time of it lately living between her grandmother and Aunty due to her mother not wanting her at home, which makes her feel unwanted. Since being on the program and support she has not reoffended for 3 months and is making an effort to re-engage back into school and make positive choices.
