The client is a 72-year-old man facing significant medical and mental health challenges. Currently, he is in the Transition Care Program at the hospital, as he does not have a stable housing option to move to. He was forced to leave his long-term private rental due to the property's sale, and he lacks family support or an alternative housing arrangement.

Initially admitted to the hospital, he was later transitioned into the Transition Care Program. They have managed to secure permanent housing, but are currently facing financial hardship, particularly with regards to their basic needs. Furthermore, the client lost all his possessions, leaving him in need of essential items like a small fridge, washing machine, bed, and a recliner chair for mobility support. He also requires a new bed and mattress, along with bed linens, to rebuild his living situation.
